Photo Consent

At Roots and Shoots Learning, we occasionally take photographs or short videos during sessions to celebrate learning, capture special moments, and share the vibrancy of our community. These images may be used in internal displays, newsletters, training materials, or on our website and social media platforms - always with care and respect.
We ask for your consent to use photographs or videos of your child/young person for these purposes. You can choose the level of consent you’re comfortable with, and you’re welcome to change your preferences at any time.

EAV Consent form

This form is only required if requested by Louise Sullivan.

Emergency First Aid Consent & Medication Administration Form

At Roots and Shoots Learning, the safety and wellbeing of every child and young person is our highest priority. In the unlikely event of a medical emergency, we may need to act swiftly to provide first aid, contact emergency services, or arrange hospital care before a parent or carer arrives. This section ensures we have your clear consent and accurate information to support your child safely and responsibly. You’ll also be asked whether your child requires any medication during the day while in our care, so we can plan and administer it appropriately if needed.

Reporting Bullying Anonymous Form

At Roots and Shoots Learning, we do not tolerate bullying in any form. We are committed to fostering a culture of kindness, respect, and safety, where every child feels valued and supported. Our anti-bullying policy outlines how we prevent, identify, and respond to bullying, and how we actively promote positive behaviour across all settings. Although we would encourage parents/carers to discuss with us directly, if you do have concerns, and you don't feel able to contact Louise or any team member in the first instance, you can use our anonymous bullying report form to share them - no names, no judgement, just a safe way to help us protect and support every child.

Travel Consent

From time to time, Roots and Shoots Learning may use an alternative base for sessions to enrich the learning experience of our children and young people. These off-site opportunities are carefully planned to support engagement, exploration, and growth in new environments.
On these occasions, staff may use their own vehicles to transport students. All staff vehicles used for this purpose are fully insured for business use, and we maintain up-to-date records to ensure safety and compliance.
